How large should I suspect to be able to go?

Hello! I am a transgender woman heavily considering getting breast augmentation. My breasts are quite small at the moment, I have been on HRT for roughly 2 years. Getting large breasts is my main goal for the procedure. What sorts of factors will determine how large I could go? I'm 5"11' and 140 pounds. Thanks!

This question will probably be answered differently by each surgeon. One factor you should understand is, the bigger you go from an implant standpoint, the more likely you are to have the implant move into a position that does not look right, also known as "implant malposition." You will also be at higher risk for forming stretch marks, needing a breast lift in the future and other complications.

After understanding the risks, if you want to go larger, then I would start with reasonable sized implants (350ml or so) and at a later date, switching to larger implants (500ml or so). Good luck.

Thank you for such an excellent question and welcome to our world of Plastic Surgery. I am the only openly transgender member of this society and have treated many patients like yourself. Choose a surgeon who is a member of this society or candidate member. But also be sure that surgeon is transgender friendly. You will find surgeons who do not want you in their waiting room. This is a simple operation that can be performed in your community. You do NOT have to travel to a “transgender center.” You are no different than a cis gender lady with small breasts. First I would recommend you wait and gradually gain weight with proper foods and exercise. Your Body Mass Index is only 19.5 and this is considered underweight, not good for wound healing and a “healthy look.” Regarding size I suggest gel implant of moderate size to avoid “stuffed look.” 1-2 years later you can go much higher. I urge you to consider a Brazilian Butt Lift as well, this will highlight your breasts and butt.

It is best to answer your question during a face-to-face evaluation, when you can discuss your goals and expectations for the procedure, and you can have a physical examination to evaluate for that procedure. I recommend that you find a board-certified, or board-eligible plastic surgeon with whom you are comfortable. Be sure to have all of your questions answered during a face-to-face meeting with your surgeon, and review before and after pictures of similar patients whom have had this procedure.
