If it is just the nipple and not the areola that is sticking forward, the nipples can be reduced but trimming them. This is an office procedure that is done with just a local anesthetic in less than a few minutes.
You can shorten the nipples. Just take a ring of tissue off at the base and then sew the top down to shorten the projection. It was a skyscraper you just take off the bottom 10 floors to lower the skyscraper

If it’s more of a “puffy nipple” or protrusion of the areola it’s probably due to excessive breast tissue under the areola. Surgery is still minor, but you might want oral or intravenous sedation.

This can be treated in mild cases under local, or mild sedation, in the office by removing this tissue by an incision in the areola or colored portion. In more severe cases gynecomastia surgery is done. If just the size of the nipple itself, that is rarer in men, but can be treated by excising a portion of the nipple by several different methods depending on the type. This also, can be done under local, reducing the cost.
