I am a 6'8 tall transgender with a 44-B bra. I would like to have D-DD to fit my frame, and my height. I don't think that is too much to ask considering I see other transgender women with much bigger. I'm not looking for extreme implants, just something that fits me. What would you recommend? I really like silicone over saline.

It is best to answer your question during a face-to-face evaluation, when you can discuss your goals and expectations for the procedure, and you can have a physical examination to evaluate for that procedure. Breast implant sizing is determined by a number of factors, including your chest dimensions, current breast size, skin quality, and your desires. I recommend that you find a board-certified, or board-eligible plastic surgeon with whom you are comfortable. Be sure to have all of your questions answered during a face-to-face meeting with your surgeon, and review before and after pictures of similar patients whom have had this procedure. It is possible to get to the size implant you want , however, if your skin is tight and thin.. you may need to do this in several steps
Consideration for thin skin can be the use of fat grafting in addition to the implant , or a hybrid breast augmentation. Of course tissue expanders are an option but are expensive and serial augmentation probably a better option overall

I recommend that you seek an in-person consultation with a board certified plastic surgeon to review your goals and discuss implant options.
