Explant and fat transfer to breast

I currently have C cup saline implants. I got them about 10-15years ago, I was a size 32a. Since than I have had 3 kids since. I didn’t breast feed much 1-2 months max. Would I be a good candidate for a fat transfer to my breast instead of implants? If so what cup size range would I have the potential to be?

Assuming your current implants are 400cc or less you would need 1000cc of liposuction conservatively speaking to get the required amount of fat. Without knowing your height and weight it's impossible to guess if that amount of fat can be harvested from elsewhere. Having had breast fed 3 kids there is a possibility that you would need a mastopexy, so fat grafting alone may not be a good option. Lastly fat grafting is not equivalent to or an exact replacement for breast implants. It's impossible to know how good or even if you are candidate for implant replacement with fat grafting or what size you could attain with only fat grafting from the little information in your post.

Hi Lisa. There are many factors to consider when considering explant surgery.

#1) how much of your native breast tissue you currently have

#2) What degree of breast sagging you have

#3) Whether or not you have an indication to have your breast capsules removed along with your implants (i.e. capsular contracture, Breast Implant Illness, or concern regarding certain textured devices)

#4) How many fat donor sites/extra fat (that can be liposuctioned) you have, and how much usable processed fat those will provide

I encourage you to come into the office for an in-person consultation ph 310-777-5400 to help determine the best surgical plan for you. I have extensive experience with these operations, and also have a Breast Team of ancillary specialists available to my patients for other issues that may come up during your recovery. My website has a wealth of information about this exact process. Alternatively, please visit my Instagram page @kevinbrennermd or @explantliaison for photos.

Thank you for sharing your question. Unfortunately without photographs or an in-person examination it is difficult to offer advice. In most cases fat grafts to the breasts will yield a cup size per transfer but this will depend on the amount of native breast tissue, and degree of skin excess/sagging that you may have. best to seek out consultations to get the best options.

No. Fat tansfer will only increase your cup size by about one cup size and will not help much if your breasts have dropped after your children.

From my experience and published data, the average amount of size from fat grafting is one half to one cup. About 20% of patients will see even less or none, due to unknown factors that affect the ingrowth of blood vessels that nourish the fat. It is genetic, so it is on both sides, About 20% will get greater than one cup size. That assumes you have sufficient fat, you need at least 500 cc removed to attempt one cup. Although it can sometimes be done at the same time as implant removal, ideally it is done later to allow more layers to safely inject the fat as well as to correct contour deformities from the previous surgery . If you want more than one cup, you should consider another implant.
