Failure in platysmaplasty & neck lift procedur

Had a neck lift and platysmaplasty 2016 by a ASPS surgon in Plano TX to get rid of a minor double chin, my double chin got much worse. A big hollow between the platysma muscles and a lot of extra skin showed upp after 3 months. He performed a revision in 2017 and one in 2018 with almost no result. Look awful and ugly, hate myself, hate to look at the mirror. Please advice.

Based on the pictures posted and limited information f the type of surgeries was actually done.

It seems that too much submental fat was removed resulting in irregular areas of fat removal and possibly scarring of the skin to the underlying structures. need to examine in person to assess the amount of scarring and this will determine the type of corrective surgery.

Need to do fat transfer to the areas where too much fat was removed. This may also improve on the scarring and the quality of the skin. But the fat transfer needs to be done several times three to six months apart. Then reassess the area and may require a formal neck lift. It requires patience and close partnership with your plastic surgeon and multiple surgery to achieve improvement and not perfection.

Thank you for sharing your question and photographs and I am sorry that your results have not been what you anticipated. It appears that you have had scarring take place between your skin and underlying neck musculature as a result of too little fat. Likely the best treatment will involve scar release and fat grafting but a thorough review of your surgical procedures needs to be done first.

Hello, thank you for your question and I'm sorry this has been just a difficult and unsatisfying process for you! It appears from your photos that too much fat was removed during the original platysmaplasty - this happens with overaggressive excision superficial and deep to the platysma muscle. There are a couple ways this can be addressed which may include fat transfer to the hollow and revision lateral neck lift for the skin or even direct excision of the skin under the chin if you don't mind a scar in that area. The trouble with fat transfer is that in areas that have scar it may take several sessions to restore volume and improve scar quality, it is a wonderful adjunct to what we can do surgically though!
