my case

i'm a 43 years old male height 173cm weight 109 i have an excessive pubic area fat covering my penis i went for a local plastic surgeon and he said can not do the operation because of my weight although i did a bariatric surgery two years ago and lost almost 60 KG and now my weight won't go down below 109 kg, what is your advice, this condition i have is embarrassing me from getting married .

This is Buried penis (penis partially or completely hidden below the surface of the skin). It can be complicated by skin rashes, intertrigo, recurrent urinary tract infections, voiding dysfunction (hesitancy, poor flow, postvoid dribbling, incontinence), erectile dysfunction or pain with erection all of which would require medically necessary surgery. The recommended surgery is

1. Scar/dartos band release with or without suspensory ligament release

2. If needed removal of overhanging abdominal tissue by panniculectomy, suprapubic lipectomy, abdominoplasty or escutcheonectomy

3. Penile fixation - suture tacking the tunica albuginea to the ventral subdermal dartos to prevent retraction of the penis into the fat

4. Removal of scarred stiff shaft skin with direct closure, z-plasty and or skin grafting

You need a urologist for the penis part and plastic surgeon for the escutcheonectomy+liposuction. Based on the photo your current weight is not an issue.

Looks like you have excessive skin and fat covering your penis. The skin and fat can be removed with a delicate surgical procedure. Good luck..

This is a fairly simple condition to treat. A Monsplasty removes the excess fat/skin of the mons area to flatten this area and to expose the penis. This can be combined with an abdominoplasty/panniculectomy for maximal skin tightening of the abdomen as well.
