Would Columellaplasty work here?

I had a rhinoplasty about 5 years ago, and, when you are looking at me, the tip of my nose dips down further than the nostrils of my nose. This is made even worse when I smile. I hate it. I already have a short upper lip, and this makes it worse. I don't want to do another full open rhinoplasty, but I see that there are closed columellaplasties that are much less evasive. A fit for me?

Drooping of the nasal tip when smiling is usually the result of the action of the depressor septi muscle. This can be addressed by a limited incision in the nasal aspect of the columella. It is not alway successful as the muscle may reattach and renew its action.

Botox works fantastic for the depressor septi nasi: one shoot and you will be fine for up to 6 months, otherwise with local anesthesia can be fixed easily and permanently.

I suggest that structural fat graft in the columella may lengthen your columella , and also fat graft in the nostrils to make the nasal tip less prominent.
