Gynecomastia is often an inherited condition that cannot be improved without surgery. The condition can often be corrected with liposuction by making a small incision under the nipple. You'll probably feel some pain when you wake up, especially if you cough, and you will likely have a few days or weeks of breast tenderness. You will have to wear a surgical garment for a week, and you should avoid any jarring exercise for at least one month.
