Question about modified buttock lift after weight loss proceedure-cannot find before/after pics

Hello-I am a 62 yo woman-had 25 lb weight loss 2 yrs ago, kept it off. I am fit, workout 4 times/wk and do not have a large amount of fat to harvest. I am 5-5 and wt:138. My biggest problem is sagging underneath buttocks. There are 3 folds insteadof the usual 1 that most people have. Havebeen to 2 plas surgeons and they always steer towards360 BBL. I only want liftandpossible fat trnsfer pls advs

Hello, the optimal thing is to evaluate you, and due to the distance we can make an online consultation. According to your photo, in principle our suggestion would be a buttock lift in butterfly wings. The fat transfer will be subject to the donor area that impresses little. For which if you need or want to increase the volume or projection of the buttock we could perform a dermo-fat buttock hang. If you are interested, let's arrange a consultation. The best option for a weight loss patient would be a flap augmentation with fat transfer. We routinely do this for our weight loss patients. The flap is lower back/upper butt fat that is used to augment the buttocks. Additional fat transfer helps in creating a rounder shape. If you can pull up on the area just above your buttocks and the excess folds disappear, then an excisipnal buttocks lift would likely help. You would have full wing scars above your buttocks, but these would be covered by your underwear.

the best way to individualize your case would be through a consultation in person or online. You are most likely a candidate for lover body lift with or without liposculpture.

Dear Jill,

An upper buttock lift is not likely to transmit all the way down to your lower buttocks, even with the flap augmentation. An option might be direct removal of skin along your lower buttock creases. Fat transfer might then enhance the results. Good luck!

Thank you for sharing your photo. You are doing a great job with exercising and maintaining a good weight. Your weight loss and normal aging both contribute to the mild skin sagging that you have demonstrated. Liposuction would not be the primary treatment rather a "lift" of some type...likely a buttock lift or "lower body lift" is likely what you should consider. While it is a much more effective treatment for your concern it is a more major procedure with longer recovery and some plastic surgeons may be steering you to a lesser option as they may not wish to take on a more major procedure.

You already had two opinions and I hope they were ABPS certified or eligible plastic surgeons. For anybody to give you advice you need an in person exam and evaluation. If you doubt what these two told you then you should try to another surgeon close to where you live. I have no idea what a 360 BBL is.

From your given photo the recommended treatment for you are buttock implant to improve volume up with additional fat transfer helps in creating a rounder shape for your buttock and get rid of lower buttock skin folds.

Congratulations on your weight loss! I would recommend you consider 2 things - ultimately it would depend on the overall goals and your shape (cannot really evaluate the lower back/upper buttocks in your photo). Option one would be a true buttock lift, option 2 would be a thigh lift in the groin crease - I believe given what you've stated that this would address your issue best. To really give you the best answer would require an in person exam or pictures showing the full lower body. I do offer virtual as well as in person consultations.
