I've been searching the internet for months looking for a surgeon who can remove my breast implants, perform a Crescent Mastopexy, and fat grafting to my breasts with minimal scarring. My breasts are beginning to sag, since I stopped my breastfeeding journey. I have also become increasingly paranoid about BII. Can anyone recommend a skilled surgeon or have experience with this procedure? Thank you

I think most plastic surgeons are capable of performing this operation. The question is if it is the right choice for you. My colleague, Dr. Gutowski, is a really good surgeon if he is close to you. Be sure you listen to your plastic surgeon.
Agree with the comments above. It is not a question of who can perform the procedure, but whether you are a good candidate. I can tell you I have not encountered a patient yet who is a candidate for a crescent lift after implant removal, but it depends on your age, skin quality, breast shape, and implant size, among other factors. If you have sought several opinions and they have advised against it, perhaps you should heed the advice rather than searching for someone to do what you ask for - because you will eventually find one! I do implant removal frequently along with breast lifts. In most cases, a crescent mastopexy is not enough as it only provides millimeters of lift, and changes the size of the areola from round to oval. If your areolas are oval in shape and you already have a periareolar incision, then it may be enough. Fat grafting can be done at the same time as removal of implants and a lift. It appears from your post that you are worried about your scar of mastopexy. After an implant removal (as everyone has suggested crescentic mastopexy is not an option. Usually the laxity that appears after implant removal help formation of good scar. Please remember that visible scar after an operation on the breast are around NAC which you agree to by proposing 'crescentic scar' and that portion of the breast which extends from lower part of NAC to the point where the inferior part of the breast folds in. A year later, it is negligible.
What is more important is the shape of the breast. Let your surgeon decide. There are many good surgeons who can do it to your satisfaction.
If I was you, I would not travel anywhere to have any of this done. There are many good plastic surgeons close to where you live. Instead of looking in the internet for someone that call themselves experts at this procedure you need to have a consultation with a local ABPS certified or eligible surgeon close to you and have them tell you if you are or not a candidate for this procedure. The crescent mastopexy is usually not a good procedure unless in the perfect candidate of which there are not many. You can not do surgery without scars. The chances of you getting a breast cancer are about one in seven or eight with or without implants and the chances of you getting a cancer from the breast implant are infinitesimally small.
Breast implant removal with breast lift with fat transfer is possible but candidate for a crescent lift after implant removal, but it depends on your age, skin quality, breast shape, and implant size, among other factors.
