The circulation to the skin is compromised and your surgeon needs to determine if debridement is necessary. Surgical reconstruction will be necessary in the future.

You have lost some skin (the areas of depression and yellow tissue. The areola-nipple area has a scab. One can not tell how compromised the areola and nipples are in certainty due to the dark scab. Once the scab falls off, your surgeon can evaluate. I have seen some cases where the tissue recovers. You do need immediate medical attention by your surgeon.

I just reviewed the picture, thank you for uploading it. The black area indicates necrosis or in other words the skin did not survive. It is hard to say how deep this is. Sometimes it is only the outer layer of the skin and sometimes it goes much deeper. At the present time, you need close follow up with your plastic surgeon for wound care and possibly may need a revision surgery in the future. Best wishes

Hard to tell how much tissue loss there is but appears the nipple is lost. The wound needs to heal before anything else is done. It may need a little debridement but should be left to heal by itself. I would treat it with soap and water daily along with Silvadene dressings. It may take a couple of months but it will heal and will get smaller. If available and cost is not a problem then the use of a negative pressure device will make it heal a lot faster, perhaps 2 or 3 weeks at most. Then you let the wound mature for 6 months to a year at least and consider other options for reconstruction or you may just want to leave it alone.
