I’ve had a deflation of a 325cc saline implant that was 12 years old and am torn between removing them vs replacing. I’m a runner and very active . I’m 42 and scared that if I remove them I’ll be very unhappy with how much they sag or scared they will just look horrible. I am 5’8” and 115lbs. Not sure I want to keep having surgery every 10 yrs either. What do most women end up doing?
I think most women have them replaced but you should not base your decision on what other women do. You don't say how long ago your right implant deflated. To me, it doesn't look a difference of 325ml between the two sides, so there may still be some saline in the implant. You don't need to have surgery every ten years. While implants should not be regarded as permanent, you can keep them as long as you are not having any issues. If you do replace them, be sure to get smooth shell implants.

THis is a great question and one we get asked all the time. This is highly individualized as every patient has different goals as to what their ideal shape and size of the breasts are. It is very important to meet with a board certified plastic surgeon who will do a physical exam and discuss with you all advantages and disadvantages of all of the different options of removal only, removal with a lift, removal and replacement with a lift. In my office we have 3-D imaging which helps. Its not an end all be all but it helps both the surgeon and the patient be on the same page regarding the patient's goals as well as realistic expectations.
The great thing about saline implants is that it gives you the opportunity to see what your breast will look like after removal. Over the following months, ask yourself if you prefer the deflated side or the implant side. Either is a perfectly acceptable option for your age and activity level.
