Breast cancer reconstruction

Skin/nipple sparing bilateral mastectomy w/placement of expanders under muscle 1/29/19 w/ 200cc during surgery. Expanded to 425 now, DIEP Flap scheduled in May but have developed scar tissue on ribs around expanders. I’ve been asked to consider implants but worried about more scar tissue & didn’t want anything foreign in my body, what other reasons should I consider to make decision?

It sounds like you've been through quite a lot. Cancer reconstruction can be complicated, as you are finding out, and it's hard to give you any good information without actually seeing you in person. This question is really better answered by your surgeon. He/she understands what's happened so far (e.g. why do you have expanders if you were planning to have flap reconstruction). And don't be afraid to get other opinions!

Thank you for sharing your excellent question. In many cases the scar created from the tissue expander can be left in place, or removed at the time of your DIEP flap. Due to complexity of reconstruction, and without knowing your full medical history, it is difficult to offer definite advice so see a series of surgeons in consultation to discuss what options best help you attain your goals.

Why did you have expanders placed and not a flap at the time of your mastectomy? Did you have radiation after your mastectomy? Do you currently have capsular contracture? These and several other issues are important to know to help determine your best plan. With expanders in you have several options and there are risks and benefits with each. Having a plastic surgeon who performs all types of reconstruction is important to give you the best plan.
