Assuming your surgeon is a properly trained ABPS diplomate, whatever you keep at 4 months should stay with you without any more grafting.
Once you get a bbl, and if your weight is stable, then whatever fat survives from the initial surgery should remain.

After any fat transfer surgery, a certain amount will take and some will not survive. This amount fluctuates depending on the technique used, location, and amount of fat injected. Once the fat survives, it is permanent. As with fat in any other location, it will grow and shrink with weight fluctuations, but overall the volume should maintain over time. Hope this helps!
