From your description you are high risk with significant deformity from your large hanging apron of belly skin and fat. With no further information, the best fit is panniculectomy. In these types of cases it is usually a medically indicated procedure rather than cosmetic. Best wishes and I am hoping that you can get treated soon.

Choosing the right operation for you would really require an exam in person. Your medical history is very important as well and this will contribute to the appropriate procedure. Best of luck.

You don't give your height and wight. In general: liposuctionis for those individuals at or near ideal body weight with localized fat deposits resistant to dieting who have good skin tone and in good health. Tummy tuck is for those with excess/stretched out skin after large weight loss or multiple or twin pregnancies. You need to lose weight first, either through diet or bariatric surgery . This will also help your diabetes

Without photographs and knowledge of your full medical history it is difficult to offer recommendations but it sounds as if a panniculectomy would be a good start.

Thank you for your question. It is hard to determine which procedure would be the best for you without examining you. In addition, either a panniculectomy or Abdominoplasty will have the best results when you have lost as much weight as possible and your diabetes is controlled. My criteria for any elective surgery is BMI of 32 or less and Hgba1c < 7 for diabetics.

Please consult with a board certified plastic surgeon to determine which procedures are best to achieve your goals.

Best wishes!
