Chin augmentation failure, crooked smile due to misplacement, re-positioning or fat grafting now?

3 failed surgeries in 3 years that include chin augmentation. Feared I had permanent nerve damage, think I'm ok now, but stuck with a crooked smile and a butt chin. The piece is clearly misplaced again. For solution, which one is safer, more effective, and more cost efficient? 1 - Get the same chin piece in place re-positioned? OR 2 - Have the chin piece removed and get fat grafting done instead?

I don't really do much chin augmentation, but there is a way to advance bone to give the chin augmentation. With your history, this may be a better option. Unfortunately, I don't personally know anyone in your area. You would be looking for a genioplasty.

3 failed surgeries in 3 years is very unfortunate and uncommon. I recommend discussing the possible reasons for repeated unfavorable outcomes with your trusted plastic surgeon and formulate a plan going forward. Sometimes the best plan is to remove the chin implant and allow your tissues to heal for 6 -12 months before a fourth attempt at augmentation. Fat grafting can be an option; however, this will be more successful when your chin area is soft and supple.

It is best to answer your question during a face-to-face evaluation, when you can discuss your goals and expectations for the procedure, and you can have a physical examination to evaluate for that procedure. I recommend that you find a board-certified, or board-eligible plastic surgeon with whom you are comfortable.
