Buccal fat pad excision is certainly an option if you have done lower cheek fullness. It is best to be seen in person to be evaluated. Properly.
Bichectomy is a fairly straight-forward procedure to narrow the mid to lower face profile.
However, part of the widened facial profile may be due excess bigonial width, deficient bizygomatic width, or both. In other words, you may not need a bichectomy. You may benefit from upper cheek implants, reduction of your "mandible width" via masseter reduction or even ramus shave (which I absolutely do not recommend), or possibly even orthognathic surgery. My point is that you likely have options, and some of the other options may better serve your needs. Definitely meet with a board certified plastic surgeon. Make sure he or she discusses these components. You need to determine what parts, if any, are disproportionate in profile. Best of luck!
