A 29-year old male presented to our clinic with cosmetic concerns relating to his profile, notably his “big nose” and “weak chin.” Examination revealed a large dorsal hump, under rotated nasal tip, increased columellar show, microgenia, submental fullness and blunted cervicomental angle (Figure 1a, 2a). His chin-throat distance was 2-fingerbreadths, and an inverted V- sign was also evident. The nasal speculum exam showed an S-shaped septum with deviation to the right and internal valve collapse. Significant history includes recurrent trauma to the nose, chin and face due to sport-related injuries. No significant medical or surgical history were pertinent to his presenting concerns.
