Coding for complex microsurgical flap procedures can be difficult for two reasons. First, the code set related to microsurgical flaps is quite small, and procedures need to fit appropriately into these limited codes while also respecting that some procedures may be easier – and others more complex – than then typical procedure for which the code is valued. Second, any complex procedure is often unique. Nevertheless, just because a microsurgical flap is unique, it might not mean the procedure should inherently be reported with multiple CPT codes.
