Product Description

In this video-based CME activity, Louis L. Strock, MD performs a transaxillary endoscopic breast augmentation on a 26-year-old woman. Get a first-hand look at how direct visualization of the pectoralis dissection, using endoscopic assistance from the perspective of the transaxillary approach, can be performed on a consistent and predictable basis. This highly visual learning experience will help you comprehend the surgical steps necessary to safely divide the pectoralis muscle and create the partial subpectoral pocket based on the needs of the specific patient shown. The patient's inframammary fold position is maintained without modification. The tissue release is performed to minimize bleeding and tissue staining is done during all phases of the procedure. In this case, the implants are placed in a partial subpectoral pocket to optimize tissue cover and minimize mammographic interference for the long-term result. The transaxillary approach is chosen to allow for implant placement using an incision remote to the breast.

Learning Objectives

After viewing this course, the participant should be able to:

  1. Create and strategize for management of the inframammary fold preoperatively on the basis of preoperative markings, used intraoperatively by correlation of these markings with internal anatomy visualized using endoscopic assistance.
  2. Understand the advantages of endoscopic tissue visualization, and the specific aspects of surgical technique that minimize bleeding and tissue staining, that serve to facilitate ease of endoscopic tissue release in a controlled and predictable fashion.
  3. Implement postoperative management strategies to maintain control of implant position.
  4. Understand issues of patient selection to minimize complications, particularly early in one's experience with the technique.
