An obese 61-year old female presents with a proximal tibia fracture non-union with multiple attempts at ORIF, iliac crest bone grafting, and antibiotics resulting in hardware failure. For definitive treatment of her non-union, a pedicled fibula flap was performed and the non-union was resected. Nine months post-operatively she has radiographic evidence of union and is able to weight-bear on her extremity.
