This is a case of a 40-year old female who presents for breast reconstruction salvage after multiple failed procedures. There are no abdominal-based flaps available and she firmly does not want implant-based reconstruction. Through this case, we demonstrate how using a rational approach that incorporates all available reconstruction methods can allow a surgeon to achieve a reasonable, mutually agreed upon surgical sequence to achieve well-defined goals and expectations.
