A 10-month-old female with medical history of respiratory failure, acute hypoxic renal injury and sepsis secondary to intubation-dependent chronic lung disease of prematurity was started on arginine chloride via peripheral intravenous line, 5 grams every 8hours (25mEq per dose), for management of metabolic alkalosis and hypochloremia of renal insufficiency. Twelve hours after the start of infusion, nursing staff noticed blistering at the IV site during a routine skin exam.
